What should you expect once the application is filled out and submitted?
Once the application is filled out, it will be reviewed, and your insurance will be verified. This is the application processing step.
If selected, records will be requested from the previous primary care provider. An appointment will not be scheduled without the records.
All new patients will be required to have two appointments. The first is the new patient intake appointment, and the next is your physical or follow-up appointment, depending on the situation.
All new patients are strongly encouraged to set up the Elation Patient Passport Portal upon receiving the invitation.
No medications will be sent from Access Family Medicine until you meet with your new provider.
Access Family Medicine is a small, privately owned medical practice. Please allow five to seven business days for your application to be processed.
